Entry is based on individual needs and suitability to the group home environment.

Female or male. Individual must be 18 years of age or older.

The primary diagnosis must be developmentally disabled.

Individual must be ambulatory.

The Admissions Process:

Initial Consultation Tour

Meet with our team to discuss your needs, learn about our services, and tour the group home to determine if it’s the right fit.

Comprehensive Assessment
a thorough evaluation is conducted to understand the individual’s medical, behavioral, and personal support needs.

Personalized Transition Plan
we develop a customized plan to support a smooth transition into the group home, including coordination with healthcare providers, service coordinators, and family members.

Community Engagement

At Collaboration Effort Inc., we believe in fostering meaningful connections and active participation in the community for individuals with developmental disabilities. Our community engagement activities are designed to promote inclusion, independence, and social interaction.

Activities Include:

  • Group outings to local parks, museums, and events

  • Volunteering opportunities at food banks, shelters, and community gardens

  • Attending community fairs, concerts, and festivals

  • Visiting libraries, recreational centers, and places of interest

  • Participating in inclusive clubs and special interest groups

  • Weekly social meetups and themed gatherings

  • Supporting local businesses through planned visits and shopping trips

  • Involvement in church or faith-based activities (if chosen by the individual)

  • Attending advocacy events or self-advocate groups
