Collaboration Effort Inc. is dedicated to meeting all federal HCBS requirements (42 CFR 441.301) for individuals receiving DD Waiver group home services, ensuring their rights, autonomy, and full community access are upheld.

Key Commitments:

• Community Integration: Residents are supported to engage fully with the broader community.

• Rights Protection: Ensures privacy, dignity, respect, and freedom from coercion or restraint.

• Autonomy and Choice: Supports individual decision-making in life choices and service options.

Residential Setting Requirements Include:

1. Lease agreements with legal protections.

2. Privacy, lockable doors, roommate choice, and freedom to furnish personal space.

3. Control over personal schedules and food access.

4. Freedom to receive visitors anytime.

5. Full physical accessibility.

6. Modifications only when necessary, justified, and documented in the person-centered plan, with clear safeguards.

Person-Centered Individual Support Plans (PCISP):

· Reflect the person’s setting choice, strengths, needs, and goals.

· Include culturally appropriate supports, both paid and unpaid.

· Provide for risk management and backup plans.

· Ensure informed consent and self-direction.

· Avoid duplication and unnecessary services.

Ongoing Oversight:

· Annual notification of HCBS rights.

· Reviews conducted meaningfully, with documentation.
